There is a special insurance program for international students. Of course, the insurance premium is charged together with your tuition fee, and the insurance policy covers only the basic health care. Also, in many cases there is co-pay when you are filling your prescription. The U.S. is the one (not sure if it is the only one) of developed countries which does not provide a general basic health care to all of its residents.

Of course not all. It really depends on which program you are applying to. Generally,

1) The hotter the program is, the harder it is to get the funding.
2) The richer the program is, the more funding you may be granted.
3) Usually in economic downturns, it is not easy to get funding, and universities desperately want to earn more money.
4) Chicken universities don't give any financial support.
