You'd better not expect to use one method to deal with all endogeneous problem. For simultaneity, I think when you derive structural model from theoretical model, the theory can directly tell you which factor is the cause and which factor is the consequence. Anyway, I am not an expert in this area but Toni is. So, please read her survey carefully and I think you will find the answer.
